Do Stronger Legs Correlate with a Stronger Brain?

The evidence strongly suggests---yes!
A growing body of observational research has examined how leg strength is associated with slower cognitive decline, larger brain volume, and lower dementia risk. One notable example is a 10-year study of healthy female twins, which found that greater leg power at baseline predicted less cognitive decline and better brain structure, including greater grey matter volume over the following decade. Because the study compared twin pairs against each other, it could largely rule out shared genetics and early-life environment as explanations for the association. Still, this and most of the research in this area is observational rather than experimental, so it demonstrates a strong association—not proof that stronger legs directly cause a healthier brain. Reverse causation is also plausible: people in the early stages of cognitive decline may already be moving less, which could weaken their legs rather than the other way around.
Walking speed, balance, and the ability to move from seated to standing are commonly used as physical performance measures correlated with leg strength and cognitive status. A 2024 analysis combining two large longitudinal studies found that slower gait speed was associated with 52–73% higher dementia risk, balance impairment with 58–97% higher risk, and poor performance on a sit-to-stand (chair stand) test with 56% higher risk, depending on the cohort. People with both gait and balance impairment had roughly two to three-and-a-half times the dementia risk of those with neither.
Additionally, increased leg strength and better balance can reduce fall risk, which is an important factor in cognitive health and long-term independence.
Possible Mechanisms
Researchers have proposed several mechanisms by which leg strength may support brain health, though the strength of evidence varies by pathway. Muscle-brain signaling may influence:
- BDNF (Brain Derived Neurotrophic Factor and Alzheimer’s disease)
- Neuroplasticity & neurogenesis
- Inflammation control
- Insulin sensitivity
- Vascular function
Strong legs also generally accompany greater overall physical capacity and activity levels. Regular exercise has been associated with improvements in:
- Endothelial function
- Blood pressure
- Insulin sensitivity
- Cardiovascular fitness
- Cerebral blood flow
Additionally, large muscle groups—particularly the quadriceps and gluteal muscles—are major sites of glucose utilization. Greater muscle mass is generally associated with improved glucose uptake and insulin sensitivity, which can support better glucose regulation and metabolic health over time. This is particularly relevant because insulin resistance and metabolic dysfunction are increasingly recognized as contributors to cognitive decline.
Independence and Daily Function
Strong legs allow people to remain active and maintain independence. Increased leg strength makes it easier to:
- Walk
- Climb stairs
- Stand for extended periods
- Exercise
- Participate socially
- Avoid falls
- Get out of bed, out of a chair, or off a toilet
Loss of muscle mass and physical inactivity often go hand-in-hand with a more sedentary lifestyle, which has been associated with metabolic dysfunction and increased inflammation. Chronic systemic inflammation is also implicated in neurodegeneration. Taken together, maintaining leg strength appears to be a meaningful marker of healthy brain aging and is associated with lower risk of cognitive decline and dementia.
Simple Ways to Assess Lower Body Strength
There are simple assessments you can use to track lower body strength and progress over time, including:
- Balance testing
- Gait speed
- Sit-to-stand performance
- Leg strength via weighted exercises such as squats, deadlifts, and lunges
